چکیده
CASE REPORT A 71-year-old man was admitted to the cardiac unit having had a syncopal episode whilst eating lunch. During the meal he felt nauseated, dizzy and faint and then lost consciousness. After rapid spontaneous recovery he complained of further nausea and vomited until hospital admission. There were no epileptiform movements during the syncope. For the previous year he had had almost daily presyncopal episodes associated with prolonged standing, head turning or looking upwards, but in many episodes no precipitating features were identified. Ischaemic heart disease with infrequent exertional angina had been diagnosed 15 years previously but he was not on prophylactic anti-anginal medication. A diagnosis of alcoholic cirrhosis had been made nine years previously, and he had remained off alcohol since then, with normal liver function tests at subsequent outpatient review. On examination in the cardiac unit 60 minutes after the syncope he was fit, alert and well orientated. The heart rate was 84 per minute in sinus rhythm. Blood

Benefits of fludrocortisone in the treatment of symptomatic vasodepressor carotid sinus syndrome.
OBJECTIVE To assess treatment with fludrocortisone in vasodepressor carotid sinus syndrome. PATIENTS AND METHODS Eleven patients, mean (SD) age 83 (5) years, with daily dizzy episodes and a median of five (range two to 20) syncopal episodes over a median of one year were studied. All had vasodepressor carotid sinus syndrome (> 50 mm Hg fall in systolic blood pressure during carotid sinus mass...
متن کاملRole of carotid sinus syndrome and neurocardiogenic syncope in recurrent syncope and falls in patients referred to an outpatient clinic in a district general hospital.
Carotid sinus syndrome (CSS) and neurocardiogenic syncope (NCS) are recognised as important causes of recurrent syncope and falls in the elderly. In this study the role of CSS (diagnosed with carotid sinus massage) and NCS (diagnosed with prolonged head-up tilt) in a district general hospital were investigated. Over 27 consecutive months carotid sinus massage was performed in 139 patients. Of t...
متن کاملPresence of a carotid bruit in adults with unexplained or recurrent falls: implications for carotid sinus massage.
BACKGROUND Carotid sinus hypersensitivity is a common cardiovascular cause of unexplained or recurrent falls in older adults. Effective treatment is available once carotid sinus hypersensitivity is identified. Carotid sinus massage is the only practical method for achieving a diagnosis. Carotid sinus massage is contraindicated if a carotid bruit is present. OBJECTIVE To determine the prevalen...
متن کاملDiagnosis of carotid sinus hypersensitivity in older adults: carotid sinus massage in the upright position is essential.
OBJECTIVE To assess the diagnostic value of supine and upright carotid sinus massage in elderly patients. DESIGN Prospective controlled cohort study. SETTING Three inner city accident and emergency departments and a dedicated syncope facility. PATIENTS 1375 consecutive patients aged > 55 years presenting with unexplained syncope and drop attacks; 25 healthy controls. INTERVENTIONS Bilat...
متن کاملThe origins of the sinus node pacemaker complex in man: demonstration of dominant and subsidiary foci.
To test the hypothesis that the human sinus node is capable of demonstrating multiple sites of impulse generation, we assessed spontaneous shifts in the sinus node pacemaker complex, and shifts after overdrive atrial pacing, premature atrial stimulation and carotid sinus massage. A total of 24 patients aged 59 +/- 15 years (mean +/- SD) in whom stable sinus node electrograms were obtained were ...
متن کاملCoronary artery spasm induced by carotid sinus massage.
A 60 year old man with a history of frequent episodes of chest pain and dizziness was referred for evaluation of coronary artery disease. He had no significant coronary artery stenosis at baseline coronary angiography. A carotid sinus massage was performed for evaluation of carotid sinus hypersensitivity in the patient. Both heart rate and blood pressure decreased a little, and returned to base...
